Care Manager- Telephonic Nurse - PRN Evenings & Weekends

In this role, you will be responsible for providing telephonic support and care coordination for our members, ensuring they receive the best possible care. Provide telephonic support and care coordination for Humana members on a PRN basis, working evenings and weekends. Utilize clinical nursing knowledge and skills to assess and evaluate members' health needs. Collaborate with members, their families, and healthcare providers to develop and implement personalized care plans. Monitor and track members' progress, making necessary adjustments to care plans as needed. Educate members on disease management, preventive care, and healthy lifestyle choices. Coordinate with other healthcare professionals to ensure seamless and efficient delivery of care. Document all interactions and interventions accurately and timely. Follow HIPAA guidelines and maintain confidentiality in all member interactions. Continuously evaluate and improve care processes to enhance the quality of care provided. Stay updated on industry best practices and relevant healthcare policies and regulations. Serve as an advocate for members, ensuring they receive the best possible care and support. Collaborate with other members of the Humana team to achieve organizational goals and objectives. Maintain a compassionate and caring demeanor while interacting with members. Communicate effectively and professionally with members, their families, and healthcare providers.
